Idaho/Colorado
Green bollard with white rectangular reflector. Reflector can be both short and long.
Montana/South Dakota
White diamond reflector on top. In Montana reflector is usually attached to the stick with 1 bolt, while South Dakota mostly uses two. Also sometimes found in Nebraska and North Dakota.
Nevada
White rectangle in black outline.
New Mexico/Colorado
Three circles on top.
Northeastern States
In Maine, New Hampshire and Massachusetts you can find bollards with red or green rectangular reflectors on the ends of bridges and guardrails.
Oregon
White rectangular reflector on top.
Pennsylvania
Has a black horizontal cap near the top. The bollard can be yellow.

Utah/Idaho/Arizona
This bollard with white square reflector is found in three states in different variations. In Arizona there is only 1 bolt visible on the square(leftmost photo), while in Utah there are two(in the center). In Idaho reflector isn't mounted on the very top of the bollard(rightmost photo).
